In May 2017 an 11 year old girl died after falling to the water while on the Splash Canyon ride at Drayton Manor theme park.
The Coroner carrying out the inquest into the incident has now written to operators of other theme parks requesting that they review safety at similar rides and respond with an action plan details what steps they intend to take to prevent future deaths.
Particular issues raised by the coroner included a lack of safety warnings for guests, staff not trained in water rescue, a lack of water rescue equipment and an inability to monitor CCTV adequately.
